Children & Youth Ministry Screening Form

This form must be completed by anyone age 14 and up to work with children (volunteer or compensated) at Rockville Church of the Nazarene. This form will be reviewed by our pastors and the leader of any ministry that you desire to serve in. Typically, consistent attendance at RCN for at least 6-12 months will be required prior to review. If your role includes teaching, that will require a face to face meeting with one of our pastors for final approval. Although membership is not required for all ministry positions, it is encouraged for all and is required for most teaching positions.

I understand that if I see any behavior that may be considered inappropriate from any child or youth I will immediately ask them to stop that behavior and if it continues notify the ministry leader (Required)
